Submit an Audiogram

Hear by Design offers programming services for certain OTC hearing aids.  Customers can use this page to submit their recent audiogram to our licensed hearing instrument specialists.  Our form is secure and HIPAA compliant.

Please place an order for your hearing aid first.  Be sure to include our programming service in your order.  Once your order is placed, you may submit a current audiogram (preferably within the past 6 months) using this form.  After your audiogram is received, a team member will reach out to confirm the details prior to programming.
